Medicare PTAN Application Enrollment Service for Physicians
R.J. Hedges & Associates have been helping medical professionals and facilities get their PTAN’s for over 20 years!
We understand Medicare enrollment requirements and help providers navigate the complex process correctly - saving time and money.
What PTAN do I need to dispense products such as collagen surgical dressings and how do I get one?
You need a separate PTAN to bill for Durable Medical Equipment (DME). It requires submitting a CMS-855s application to Medicare via their PECOS system.
This application costs $750, which is paid by the physician directly to CMS via PECOS.
Applications are processed by 2 CMS Contractors, known as National Provider Enrollment (NPE) Contractors: Novitas Solutions, if you are located East of the Mississippi River or by Palmetto GBA, if you are West of the Mississippi River.
On average, applications do take three months to be processed and have the onsite inspection completed by CMS.
If everything meets CMS standards and expectations, you will be awarded a DME PTAN, and then and only then, can you start billing and dispensing.
*Please Note: Any provider EAST of the Mississippi River is being handled by Novitas. They are experiencing growing pains are taking in excess of 6 months to process applications.
What does R.J. Hedges charge and how can they help?
Our fee for guiding you through the application process through approval is $2,000.
Our process includes a data collection stage via an online questionnaire that allows you to start and stop the process at your own pace.
Our service will provide necessary Policies & Procedures, support documents, forms, instructions, and checklists to prepare for the on-site inspection from CMS.
If needed, this service also includes assistance if you have any deficiencies after your on site inspection.

If you answered YES to all of the above, you may be eligible to apply for a PTAN!

Provider Expectations for CMS Applications
At RJH, we are here to guide and support your application process to obtain a Medicare PTAN, which is required to bill for DME (Durable Medical Equipment) items such as surgical dressings and wound care supplies. While we handle the preparation and offer expert guidance, your active participation is essential to successfully complete the process.
What We Do (and Don’t Do)
We do not issue DME licenses. Our role is to assist with the preparation of your CMS PTAN application, not to grant licensure. You are responsible for ensuring that you meet your state’s DME licensure requirements. Please check the NPWest DMEPOS Licensure Database and confirm licensing needs for S01 Surgical Dressings or any other DME categories you wish to provide.
Timely Communication is Essential
- You must respond to all document and support requests from the RJH team within 2 business days.
- Delayed responses will extend your application timeline.
- Email and mail from Medicare contractors (Palmetto or Novitas) must be reviewed and responded to immediately.
- Time-sensitive documents may arrive via USPS—do not discard unopened envelopes.
Understanding the Timeline
- Initial PECOS application reviews by Medicare contractors can take up to 6 months.
- Additional documentation requests may cause further delays.
- Contractor regions:

Fingerprinting (If Applicable)
- CMS may request fingerprints from providers or facility owners as a one-time identity verification.
- If you’ve already been fingerprinted for CMS in the past, this step may not be required.
Timeline will not start until RJH Receives Completed Questionnaire, Access to I&A, and Payment
We are committed to guiding you through this process and handling the heavy lifting—but your timely participation is essential. Delays in providing required documentation can extend the timeline by weeks or months. IMPORTANT: Your timeline begins only after RJH has received all the following:
