1. Do you have a Continuous Quality Improvement (CQI) in place?
You answered:
- Yes- Fantastic! You are protecting yourself and your patient files.
- No- It wouldn't be a bad idea because having a CQI allows medical facilities to record and investigate adverse events in a privileged and confidential environment without fear of litigation, professional sanctions, or injury to professional reputation.
2. Are you enrolled in a Patient Safety Organization (PSO)?
You answered:
- Yes- Hurray! Please ensure you are reporting regularly within your PSO’s guidelines.
- No- Please be aware that all documents can be subpoena and used against you in a court of law if you are not enrolled in a PSO.
3. Does your pharmacy review EQuiPP scores monthly?
You answered:
- Yes- Great! Hopefully you are posting and reviewing with your staff so they understand the benefits of this report and can focus on areas to increase your scores.
- No- Oh My Goodness! These scores can increase your ability to gain more patients because insurance companies are graded and receive star ratings. Low scores may inhibit pharmacies from working with certain insurance companies which could decrease your script count.
4. Do you have a Medication Adherence Program in place?
You answered:
- Yes- Splendid! This simple process will greatly help to improve your patient’s medication intake and thus improve your EQuiPP scores.
- No- Adherence Programs are a great way to guarantee patients receive medication on time and take it accordingly.
5. Do you have a detailed floor plan posted including room function, square footage, fire extinguishers, exits, and designation of public/staff spacing?
You answered:
- Yes- Make sure it has the square footage listed as this is a new requirement
- No- No worries! Bust out some graph paper and a tape measure and make sure to make copies and post at least one within your facility.
6. Do you maintain SDS (Safety Data Sheets) for all chemicals used within your facility?
You answered:
- Yes- Awesome! Remember you do not have to have them physically printed off, they may be kept eectronically; as long as staff knows where they are kept and has access to them.
- No- Assign this task to an employee to list all chemicals used in the facility, then search the web for these products. The manufacturer’s web pages usually have these available to the public at no cost.
7. Credentialing, do you have all the policies and procedures CVS, OptumRx, Humana, NCPDP and the other Part D insurance companies are asking for?
You answered:
- Yes- Great, you are ready to go for 2017!
- No- If you thought 2016 was tough, this year will be worse.
8. Are you completing your OIG-SAM exclusion verifications monthly on employees, contractors, Business Associates and vendors whose products are billed through Medicare?
You answered:
- Yes- Go you! Remember to retain these 10 years.
- No- Log in to OIG’s search and start running these monthly.
9. Are you saving your HIPAA and FWA training logs, and OIG reports for 10 year?
You answered:
- Yes- Someone’s been keeping up with their Compliance News!
- No- This is a requirement so you may need a bigger file cabinet.
10. Do you have a policy and procedure for patients with limited English proficiency?
You answered:
- Yes- Fantastic!
- No- Even if your staff only speaks English this is a Humana & CVS Caremark requirement. You do not need a staff member that speaks 10 languages but you do need a policy for how to help patients with limited English proficiency.
