2. Are you dispensing only FDA approved DMEPOS items?
You answered:
- Yes- Good Deal!
- No- Time to evaluate your inventory and make sure you are only buying items that are FDA approved when they are to be billed to Medicare.
3. Do you have a Proof of Delivery and all support documentation?
You answered:
- Yes- Great you are meeting the Medicare requirement.
- No- Let’s start doing this now. Remember documentation is key when it comes to your patient files; you don’t want to lose your reimbursements.
4. Are patients receiving Patient Bill of Rights and Responsibilities?
You answered:
- Yes- You have a well trained staff!
- No- Back to the copy machine! Place some copies at check out to give to new patients or include in your new patient packets.
5. Does your pharmacy use DWO's or WOPD for all DME and Part B pharmaceuticals?

6. Does your pharmacy maintain a log of DMEPOS items sold for cash and not billed?

- Yes- Perfect! Remember this is PHI, so keep it secure!
- No- Get a note pad or create a form to collect the patients name, address, phone, item name, manufacturer, and make/serial number in case of a recall.
7. Does your pharmacy retain patient records for at least 7 years?

- Yes- Perfect! Remember they do not all need to be in the patient file it is ok to archive.
- No- It’s time to bust out your paperwork storage bins/file cabinets.
8. Do you have hours of operation posted on front of pharmacy, including an emergency phone number?
You answered:
- Yes- You Rock! Make sure to include holiday hours.
- No- No worries- this is probably the easiest requirement to fulfill.
9. Do you know how to access your Local Coverage Determinations (LCDs) to determine specific requirements for dispensing a particular DMEPOS products?
You answered:
- Yes- Fantastic, you are smarter than the average bear.
- No- Understanding LCDs enables you to meet all the audit requirements.
10. Do you have a Certificate of Liability Insurance with NSC and your Accreditation Organization listed as Certificate Holders? (If you are not accredited please answer yes).
You answered:
- Yes- Make sure they are available for an inspection and are in date.
- No- Looks like you need to call your insurance carrier today. Make sure you are requesting these forms with the correct name and address for NSC and/or your A/O.
